It's a mechanic I love, because it literally ties gameplay convenience to the in-world outcome. Sure, you can kill everyone - it's easy - but if you do, the city of Dunwall becomes a plague-ridden wasteland. It's much harder to pull off a low-chaos playthrough, but (IMO) the outcome you get is much more rewarding.
I’m not familiar with it, but it sounds fascinating!
